1. You have the right to receive benefits under the Minnesota Workers’ Compensation Act.
2. You have the right to receive the medical care that is reasonable and necessary to treat your work-related injury and/or illness for the rest of your life.
3. You have the right to the initial choice of a Doctor. It is critical that you remember that Company Doctors are not on your side.
4. You have the responsibility to tell the doctor that you were injured as well as whether you believe the injury to be work-related.
5. You have the right to receive assistance from the appropriate, qualified Commission staff and, in the event of a dispute resolution proceeding, from a Commission ombudsman free of charge.
6. You have the right to hire a Workers Compensation Lawyer to help you get benefits and/or to help you resolve disputes.
7. You have the responsibility to tell your employer about your injury or illness within 30 days of the date you knew your illness might be work related.
8. You have the responsibility to fill out a claim form and send it to the appropriate Commission.
9. You have the responsibility to tell the Commission and the insurance carrier any time that your income changes.
10. You have the right to absolute confidentiality.
